Vacation-groups for disabled for Disneyland & similar destinations?

Would like to know if there are any agencies or “department within” theme parks (tourism related) that would cater to the disabled consumer? Because of special needs (flight, ground-travel, room & board and such), it would be close to a nightmare to try and figure out the details needed to make sure that the trip would be as comfortable and economical as possible…

Do you have experience with this? Suggestions?

Not expecting any break in co$ts…just assistance in getting around to facilities that are more hospitable to the handicapped (bathrooms, hallways, entrances/exits, transportation btwn areas of interest….)
